We read with interest about the CRASH-3 trial1 and congratulate the CRASH-3 trial collaborators for their outstanding work. On the basis of the results of a prespecified sensitivity subgroup analysis excluding patients who were predicted to have the worst outcome, the authors concluded that early administration of tranexamic acid within 3 h of traumatic brain injury reduces head injury-related death. However, the primary endpoint was 28-day mortality.

In patients having cardiac and non-cardiac surgery, new-onset postoperative atrial fibrillation and postoperative delirium have been associated with several adverse long-term outcomes, including increased cardiovascular, renal, and cerebral complications, worsened postoperative cognitive function, increased mortality, longer length of stay in hospital, and increased medical costs.1–3 Identifying effective therapies and modifiable factors that might reduce the incidence of postoperative atrial fibrillation and postoperative delirium is paramount.

The health of populations across the planet is in a perilous state during the COVID-19 pandemic, with more than 550‚Äà000 deaths worldwide as of July 10, 2020.1 The disease burden is falling mainly on the most disadvantaged groups worldwide and there are major impacts on health systems across high, middle, and low-income countries. In parallel with these direct health impacts, the economic effects of lockdowns are leading to an unprecedented global recession which will have ramifications well into the future.

The fight against corruption, which is defined by the global anti-corruption body, Transparency International, as "an abuse of entrusted power for private gain", is a transversal theme of the Sustainable Development Goals that the UN member states aim to achieve by 2030. The health sector is highly affected by corruption, for reasons including the increasing demand for health services, the amount of public money spent on these services, and the consider ≠able scale and complexity of this sector, which has led to information asymmetry and entry barriers to accessing health care.

The Lancet Lecture by Patricia García1 on the pervasive yet invisible hand of corruption in global health pres ≠ents a compelling introduction to corruption and the various types that exist. Corruption truly is an "ignored pandemic"2 and we have no doubt that awareness needs to be raised; however, the Lecture does not present ways of raising awareness or specify a starting point. The recommendations to expand funding and adopt an integrated and multidisciplinary approach are incommensurate with the problem facing global health practitioners, the research community, and the public.

The CRASH-3 trial1 reinforces a growing body of evidence (such as the CRASH-22 and WOMAN3 trials) suggesting a treatment effect for early administration of tranexamic acid for life-threatening bleeding scenarios. Because the evidence presented in the trial could potentially change clinical practice, its landmark status as one of the few positive pharmacological trials in the acute setting of traumatic brain injury cannot be overemphasised.4 Still, some issues concerning the trial deserve attention.

[Perspectives] Seven things organisations should be doing to combat racism
Esther Choo | thelancet.com | 2020-07-18
In the USA, the racial disparities in COVID-19 health outcomes collided with national conversations about racism sparked by the police killing of George Floyd. These events stimulated many organisations, including health-care institutions, to scramble to vocalise their stances on racial equity. The coming years will show if these statements translate into fundamental, lasting change. There are seven actions I hope to see from health-care organisations committed to addressing structural racism and building equitable environments.

_____ | popularresistance.org | 2020-07-18
George Floyd's murder, along with the murders of Breonna Taylor, Rayshard Brooks, and so many others, laid bare for the world how police are mandated by the state to terrorize people of color, especially Black people. The severe repression against protesters put this violence on display on a mass scale and showed the lengths the state is willing to go to keep its hold on "law and order." Yet, even as politicians, NGOs, and nonprofits try desperately to co-opt and pacify the movement, the most leftward expression of protesters' demands — from defunding and disbanding police departments to abolishing police a…
