The prognosis mainly depends on the development of organ failure and secondary infection of pancreatic or peripancreatic necrosis. In the past 10 years, treatment of acute pancreatitis has moved towards a multidisciplinary, tailored, and minimally invasive approach. Despite improvements in treatment and critical care, severe acute pancreatitis is still associated with high mortality rates. In this Seminar, we outline the latest evidence on diagnostic and therapeutic strategies for acute pancreatitis.

[Correspondence] Clinical endpoints are necessary in the interim analysis of REGENERATE. thelancet.com The interim analysis results of the REGENERATE trial by Zoubair M Younossi and colleagues1 showed improvement in hepatic fibrosis in patients treated with obeticholic acid, which is a non-clinical endpoint for clinical trials in non-alcoholic steatohepatitis. However, clinical endpoints and side-effects are more important than surrogate endpoints. Non-alcoholic fatty liver disease (NAFLD), the hepatic manifestation of metabolic syndrome, is increasing in prevalence as a consequence of the obesity epidemic.

However, in contrast to patients at earlier disease stages, patients with non-alcoholic steatohepatitis (NASH) and advanced fibrosis die more often of hepatic disease than cardiovascular disease.1 These patients also have the highest risk of progression to cirrhosis and subsequent liver-related mortality; hence, fibrosis stage is the strongest independent predictor of disease progression.
