[Perspectives] Seven more things organisations should be doing to combat racism
J Nwando Olayiwola, Esther Choo | thelancet.com | 2020-08-29
The previous The Penumbra column outlined seven steps that organisations can take to address racism. In this month's column, we offer additional actions likely to be helpful in creating an anti-racist climate. With these suggestions, we highlight actions that fill obvious gaps in thinking and practice in our health-care environments.

[Editorial] Research and higher education in the time of COVID-19
The Lancet | thelancet.com | 2020-08-29
The COVID-19 pandemic has propelled the research and higher education sectors to the forefront of public attention. Laboratory capacity has been crucial for diagnostic testing; experts in infectious diseases, epidemiology, public health, mathematical modelling, and economics are central to national policy making and media coverage; clinical research has been vital to improving COVID-19 management; and our collective global future relies heavily on the development of an effective vaccine against severe acute respiratory syndrome coronavirus 2.

Amy K McLennan, Anne Katrine Kleberg Hansen, Stanley J Ulijaszek | thelancet.com | 2020-08-29
Richard Horton argues that combination prevention and global health collaboration are required to address the COVID-19 pandemic.1 We agree and suggest this should incorporate further measures. Thinking closely in terms of medical solutions could create false public expectations of a return to normal, and risks closing out non-health interventions that could lead to substantial improvements.

[Comment] Acute coronary syndromes in older patients: does older age matter?
Bjà∏rn Bendz, Lars Aaberge | thelancet.com | 2020-08-29
Ischaemic heart disease is common in people aged 80 years or older, and is a major cause of morbidity and mortality in this age group worldwide. As this patient group grows, with an increase in life expectancy of up to 10 years or more, the absolute prevalence of cardiovascular disease is expected to increase.1 According to present guidelines, medical optimisation followed by an invasive strategy–including coronary angiography and subsequent revascularisation with percutaneous coronary intervention or coronary artery bypass graft–is recommended for eligible patients with acute coronary syndromes witho…

Elizabeth C Smyth, Magnus Nilsson, Heike I Grabsch, Nicole CT van Grieken, Florian Lordick | thelancet.com | 2020-08-29
Gastric cancer is the fifth most common cancer and the third most common cause of cancer death globally. Risk factors for the condition include Helicobacter pylori infection, age, high salt intake, and diets low in fruit and vegetables. Gastric cancer is diagnosed histologically after endoscopic biopsy and staged using CT, endoscopic ultrasound, PET, and laparoscopy. It is a molecularly and phenotypically highly heterogeneous disease. The main treatment for early gastric cancer is endoscopic resection.

[Comment] A leap of faith for more effective mental health care
Charlotte Hanlon, Atalay Alem | thelancet.com | 2020-08-29
Traditional and faith healers (TFHs) play an important, but contested, part in the response to mental health problems around the globe. Particularly for severe mental health conditions, such as psychosis, TFHs are often the first contact when help is sought by individuals and families in many countries of sub-Saharan Africa.1 In part, TFHs inhabit this privileged position due to a scarcity of accessible alternatives, but their approach might also be more acceptable to communities because of shared world views about causes of mental ill-health and processes of healing.

Jonas Lanz and colleagues1 compared the effect of a self-expanding valve with a balloon-expandable valve for transcatheter aortic valve replacement (TAVR) in patients with symptomatic severe aortic stenosis. According to the results, the incidence of paravalvular leakage was higher in the group of patients using the self-expanding ACURATE neo device than in the group of patients using the balloon-expandable SAPIEN 3 prosthesis. However, we are concerned that this finding might be at least partly caused by differences in predilatation between the groups.

Democrats Are Officially Republicans
_____ | popularresistance.org | 2020-08-29
The Democratic Party has ended any debate or dispute about its true nature. It is a party representing neo-liberal interests and international gangsterism, just like their putative opposition, the Republican party. | Even a cursory observation of the recent Democratic National Convention proves that this assessment is correct. There were paeans of love to the late warmonger John McCain and even an appearance from his widow. A special segment was set aside for Republicans like John Kasich whose speech was used in part to beat down progressives and make clear that Joe Biden wants nothing to do with them. Not to be…

A Week Of Mass Layoffs
_____ | popularresistance.org | 2020-08-29
MGM Resorts is laying off 18,000 people as an unchecked pandemic leaves economic scars across a broad swath of U.S. industries, particularly those that rely on healthy crowds of people. | The layoffs at MGM, which amount to about a quarter of its U.S staff of about 70,000, caps a wave of job cuts and buyouts this week across a broad array of industries. | Economists warn that sizable layoffs will continue and any recovery is likely to falter as long as the virus rages and Congress doesn't come up with additional financial aid for the unemployed, as well as desperate state and local governments.

Why the pandemic crisis marks a dead-end for capitalism
Prabhat Patnaik | peoplesdispatch.org | 2020-08-28
There is a commonly-held view that the current crisis in capitalism, which has resulted in a massive output contraction and increase in unemployment, is because of the pandemic; and that once the pandemic gets over, things will go back to "normal". | This view is entirely erroneous for two reasons. The first, which has been often discussed in this column, has to do with the fact that even before the pandemic, the world economy was slowing down. In fact, since the financial crisis of 2008 following the collapse of the housing bubble, the real economy of the world had never fully recovered. | Small recoveries were…

The corporate Democrats are once again running as the Not-Trump Party, the second consecutive election in which they have succeeded in suppressing every issue except the fitness for office of one very wretched man. The only way a party wholly-owned by oligarchs can deflect attention from its own culpability in dragooning its constituents into a Race to the Bottom amid never ending war, is to set up a straw man to be knocked down, leaving the machinery of racial capitalism and armed-to-the-teeth imperialism intact — Hillary Clinton's gambit in 2016.

The conflict of interest between clinical trials of drugs that determine their effectiveness and safety and the pharmaceutical industry that funds most of the trials is astonishing. In other words, the regulator is financed by the entity that it is supposed to regulate. | "Human Drugs regulatory activities account for 33 percent of FDA's budget; 65 percent of these activities are paid for by industry user fees (Oct 18, 2019)." — |

Nuclear Power In The Green New Deal?

Over the last few years, there has been a growing interest in a Green New Deal and there are many versions proposed in different countries. The Sturgis and Standing Rock Protests
David Mattson | counterpunch.org | 2020-08-27
I grew up in the Black Hills of western South Dakota on a small ranch roughly 30 miles away from the small city of Sturgis and between 100 and 200 miles away from the sprawling Pine Ridge and Cheyenne River Indian Reservations. David Taylor, who homesteaded our ranch, created a sinuous inholding within the newly created Black Hills Forest Reserve, laid out to capture as much bottomland pasture as possible along aptly named Hay Creek.
